If you’ve been wearing hearing aids for years and still feel like something’s missing, you’re not alone — and you’re not imagining it. We see this constantly at Oracle Hearing Center: patients who come in with perfectly good hearing aids that were never verified, never fit precisely to their ear canal, and never actually reached their prescriptive targets. It’s entirely possible to have the right hearing aids and still not be hearing your best.
I’ve been thinking a lot lately about the gap between how we operate as a practice and how other places do. It’s easy for people to assume every audiology office functions the same way. We don’t. We absolutely don’t. In this office, we use best practices to fit and verify hearing aids prescriptively — and that difference shows up in real outcomes.
I cannot stress how often people come in to see us wearing hearing devices from other places and tell us:
- “They don’t work.”
- “I don’t hear well with them.”
- “I hear extra sounds.”
- “They don’t fit well — they don’t stay in.”
- “They’re too loud, or too soft.”
Here’s what actually goes into a hearing aid fitting that works, and why so many patients don’t get it the first time around.
What We’re Really Measuring
When we program a hearing aid, we’re not just turning up the volume. We run something called real-ear measurement — placing a small probe tube in your ear canal to measure exactly what sound is reaching your eardrum at soft, conversational, and loud volumes, across every frequency you hear.
Based on your specific hearing loss, we set targets: the soft speech target, the conversational speech target, and the loud speech target. The goal is simple — your hearing aid’s actual output should land at or near those targets across the board. When it does, speech sounds clear instead of muffled, and you’re not missing the soft consonant sounds that make words easy to understand.
Where Things Go Wrong
Two of the most common culprits we find in patients who’ve been struggling with hearing aids for years:
Feedback testing cuts into the gain you actually need. Every hearing aid runs a feedback test to make sure it won’t squeal. But if the dome tip in your ear is too open — meaning sound is leaking back out instead of staying where it needs to be — the hearing aid’s software sees that leak and automatically pulls back the volume to stop the squeal. The problem is, it often cuts out gain you genuinely need at critical speech frequencies, not just the excess that was causing feedback.
The physical fit matters as much as the programming. How the hearing aid is coupled into your ear canal has a huge effect on what you actually hear. An open dome that works fine for one person’s ear canal might be leaking sound for another. In those cases, moving to a more closed dome — or even a custom shell — can trap the volume where it belongs and close a gap that open domes simply can’t fix, especially in the trickier mid-to-high frequency range.
Everything That Goes Into a Successful Fitting
Verification and physical fit are the two issues we catch most often, but they’re really just two pieces of a much bigger picture. A hearing aid fitting isn’t a single appointment — it’s an ongoing process. Here’s everything that goes into doing it right, from day one and every visit after:
- Electroacoustic analysis in our office, confirming the hearing aids meet the manufacturer’s own specifications before they’re ever programmed for you
- Real ear measurement, placing a probe tube into the ear canal to confirm you’re actually reaching your soft, conversational, and loud prescriptive targets
- Choosing the correct hearing aid for your hearing loss and lifestyle needs — not just what’s popular or what’s cheapest
- Choosing the correct ear coupling, so the acoustic and physical fit match your ear canal and your degree of loss
- Programming with the correct settings to match your hearing and how you actually use your ears day to day
- Checking for wax blocking your ear canals or the sound outlet of the hearing aids themselves
- Retesting your hearing thresholds, speech-in-noise ability, and hearing perception over time, not just at the initial fitting
- Checking for firmware updates on a regular basis
- Connecting your hearing aids to your phone for Bluetooth streaming and app use
- Checking receivers, microphones, vents, and batteries regularly to make sure everything is functioning as it should
- Checking moisture levels inside the hearing aids, since moisture can quietly affect volume, clarity, and output
- Making sure the hearing aids aren’t feeding back around soft or moderate environmental sounds
- Fitting assistive listening devices when speech-in-noise scores show you need more support than the hearing aids alone can provide
What Getting It Right Looks Like
When we reprogram a set of hearing aids properly, we’re checking that:
- Soft, conversational, and loud speech all land at or near their targets
- Maximum output stays comfortable — not uncomfortably loud, but not so compressed that it’s holding back sound unnecessarily
- The earpiece is acoustically matched to your ear canal size and the severity of your hearing loss
- The strength of the speakers/amplifiers (receivers) is appropriate for your decibel loss across each frequency
When all of that lines up — and stays maintained visit after visit — patients consistently tell us it’s the first time hearing aids have actually worked for them. Not because they needed new devices, but because the ones they had were finally programmed, fit, and maintained the way they should have been from the start.
